In fact the screw holes fit perfectly, so either he didn’t have a 4th Gen or the 4th Gen he thought he had was a third-rate knock off. 5 stars for the finish, fit, and overall look of this item. The Google Nest design team did an appalling job with their sickly-gray wall plate, which was the wrong color for an “obsidian” thermostat and the ugliest-shaped wall plate I’ve ever seen. (Cf the Bauhaus: form follows function, not the other way round.) If aesthetics matter to you at all, this wall plate is a class act and I highly recommend it.

holes don't match 4th generation Nest thermostat, I had to make separate holes right next to the holes come with it, luckily get away without wrecking. Otherwise, it fits well
